This form is provided for PCC Members to place a reservation to have medicine packaged for purchase online. To use this system you must already be in the Patient's Choice system and you must provide your PCC member number and a valid e-mail address. When your reservation has been filled you will be notified via email that it is ready for pick-up any time before the end of the business day. Reservation requests recieved after hours will be filled/replied to early on the following day.
