Patient’s Choice of Colorado (PCC) currently offers two types of memberships: Full and Guest membership. Full membership allows access to massage therapy, acupuncture, and all classes, free of charge. We also have discounted dispensary rates listed in red (create a link to pricelist on the word red). Full membership allows Patient’s Choice of Colorado to be your caregiver through the Colorado Department of Health. We welcome all guests who already have a caregiver and ask for a one-dollar membership fee that last for a year. Your guest membership allows you to make purchases at guest member prices, and have access to our therapeutic services and classes at the corresponding rates. Guest prices are listed in black (create a link to pricelist on the word black). If at anytime you would like to upgrade to a full membership, you will need to fill out a CDPHE Medical Marijuana Registry Care-Giver form designating a PCC staff member as your caregiver. You will be given a stamped and addressed envelope and need to mail it yourself. You will also need to agree to follow Patient’s Choice of Colorado “Member Guidelines” (create a link to member guidelines) and our “Code of Conduct.” PCC assumes the right to terminate your membership at any time for any reason. We welcome all registered Colorado medical marijuana patients to join us at Patients Choice as full or guest members. If you have not yet toured our facility, please call (303) 862-5016, to set up an appointment to stop by for a visit. We look forward to serving you. All future members will need to bring the following: Valid and up to date Colorado State Medical Marijuana card, Or A physician certification that is dated within five months. And A State Issued Identification or U.S. Passport for verification of Identity. If using U.S. passport, verification of Colorado Residence will also be needed such as a utility bill. |
